个人感觉，有一些细微的区别。

同样作为man的不完全形态，lad一般指小伙子，boy一般指男孩子，年龄段上的细微差异。lad比boy要略微接近男人，尽管他们有所重叠，但用lad称呼一个人，一般带有某种期待，期待他拿出男人的气概，而用boy，多带有某种宠爱，对孩子不能要求太高。当然，也可以不加区别的使用，但是如果没把握，还是用boy别用lad了。

哪儿的人都会用boy，但lad给人一种英国的味道，略微古老的味道，比如英国口音的人说的更多，包括英国人，早期的美国人~
